If you make an appointment at Bangkok Hospital via email they will confirm the appointment in writing. They won't give you a list of treatments and dates until you are seen by a doctor.

A few things.

I applied in NY and only provided the Articles of Incorporation which shows me as an officer and my IRS EIN letter. I did not provide any further info other than a letter outlining my name, job title, my responsibilities and my authorization to work remotely as a corporate officer. I provided no further info about my company.

As it regards the bank statement they will probably reject the corp bank statement as they want to see money in your name. Also a screen shot will not be acceptable. It's easy to modify the info on the web page if you know what you are doing. Has to be in your name and should be a PDF. I just supplied a summary from Schwab without transactions.

You should be fine unless the system shows you are presently in Thailand. But still...Should not be an issue you just use your DTV on the bounce.

Been well covered. Has to be in a liquid bank account. And the applicants name must be on it. Not a corporate or business account

DTV is running about 4 weeks at NY embassy.

A friend got his in almost 5 weeks. We both were requested to supply additional info 24 hours after filing then applications were in pending status for the balance of the time.
