One clarifying question: when you say “print the data page of the old passport,” do you mean just the pic/info page or should I have a copy of the initial DTV stamps as well?

I don’t have the DTV yet. It’s a question of whether I wait to get it until I get a new passport. But it will be an eVisa

I actually think the stamp is correct. DTV isn’t a “multi-entry” visa, per se. It’s a 180-day visa that allows for exit and re-entry, but that doesn’t reset it. You have to apply for an extension, which can only be done once. At least that’s my understanding.
