TLDR : Answer Summary
An Italian national in Nakhon Phanom with a 90-day O visa expiring on February 3rd has several options for extension. He cannot extend his current visa for another three months but can pursue a tourist visa (60 days) with a possible 30-day extension, or, if he has Thai family, apply for a 60-day visiting family extension. It is not necessary for him to travel to Vientiane; he can handle his immigration needs at the local Immigration Office.
if he's married to Thai he can provide the documentation and his wife is with him then he can do this. Just make sure that's a border open to foreigners as not every one is.

Normally people who get the non-O visa are planning to apply for the 1-year extension from that.
There's no other option that will give you an additional 3 months. You'll need to leave Thailand and get a new visa, or return without a visa. But if you return without a visa you'll only get 30 days (+30 day extension possible)
thank you, that is what I told him, this would leave him 6 days short, I advised that Vientiane is the nearest place to do another 90 days but I'm guessing as there maybe somewhere closer, any knowledge would be grateful accepted
probably the closest. He'll need to make an appointment though. Maybe apply for a tourist visa and get 60 days on entry then can apply for a 30 day extension.
