I have letter from Thai hospital, in Thai, and they didn't care at all....
this is happening all over. What you could now is write that explanation, and send it to your embassy along with the doctor note to request a supporting letter from your embassy. They have the power to issue you one

I am in very similar situation and I have visited them last Friday and I have been told "no letter from embassy, no help, no rights to extend".
