After my AC Joint surgery I would need further treatment. My hospital adviced me to do the medical visa extension (Got the letter from doc). Anyone knows how to apply for that? Does anyone have a Website or Checklist for required documents?

What you'd apply for is an extension based on medical reasons.
It can be granted for up to 90 days at a time.
The letter from the hospital has to say how long they require you to get treatment or you'll just get a 30 day medical extension.
The documents from the hospital are what you require and you just go to your immigration office with copies of your passport data page, your current visa/entry stamp, a filled out TM7 application for extension and a passport sized photo. The extension is 1900baht.
Know that at some offices it's NOT an easy extension to get issued. If they think you can travel AT ALL, they can/will deny you. The wording of the hospital letter is critical
