Does it need to be from a government Hospital or from doctors Clinic

1) If you are in Thailand on an o-a visa can it be converted in Thailand to a visa based on retirement.

2) do you need background check for the o-a visa? And medical report?

I am planning to move back to Thailand early next year. I lived in Jomtien/Pattaya for several years and got by with Visa runs. Now I am applying for the retirement one once I am in the country. Do I need to get a medical report or police clearance certificate to apply? Thank you.

My husband and I are Americans and will go for a visit the end of May. We are looking at visa options to apply for in America while we continue to make the required monthly transfers of our retirement income. One of the options requires a criminal clearance from Thailand and from the US. I have two questions. First, where do we go to get this in Chiang Mai? How long does the process take? The second is, can we get it from the city we maintain residency in the US or does it have to be a national clearance? Thanks!
