TLDR : Answer Summary
To apply for a DTV visa for medical treatment in Thailand, the necessary documents may vary by consulate. Generally, you will need a treatment plan from your doctor or hospital detailing the procedure and its duration. Some consulates may also accept just a scheduled appointment with a doctor. Additionally, medical certifications from hospitals stating your treatment are required. It is advisable to contact the specific consulate for their requirements and consider seeking assistance from an agent for document collection.
Ask the embassy where you will apply, they are the only ones who can give the correct answer.

Hello, for applying the DTV visa for medical treatment, you need the medical certifications from the hospitals to stating that you're receiving treatment there. If you need any help, you can contact visathailand.com
I had a letter from bumrungrad hospital saying that I had to visit my doctor every 3 months with the next appointment on date x. My agent who handled my paperwork previously gave me the description in thai which i just forwarded to the hospitals expat department. Together with proof of funds, and income sources i uploaded the maximum file size allowed lol. I thought the more documents the better. Thatโs what i fid in the past even for tourist visa. Overwhelming them with papers and they never asked for more. I was still happy to have the agent collecing all documents and submitting them to the correct place. Expensive but more convenient. Could have done it alone for cheap, but the information i found was not always consistent and a bit confusing
Some consulates now require a treatment plan from your doctor or hospital outlining what procedure you're getting and how long it will take to get done
BUT
Some consulates will give the DTV soft power "medical" visa with just a doctor appointment booked for a specific thing
Its totally consulate dependent
What consulate will you be applying for the DTV at?
what the consulate wants is one thing. There are suggestions that the visa requirements for subsequent entries will be scrutinised by immigration, so the visa holder will need an actual plan that they can prove. We don't know this yet, but we will find out in the coming months.
And who pray tell Gerry Whelan are those suggestions coming from ? ๐
There are people who already have DTV's and entered multiple times on them already (they travel extensively) and each time they came in they were stamped in fro 180 days without questions, without hassle, without anything
I've been looking for clarification regarding 90 day reporting and the DTV. It doesn't mention it anywhere where a DTV applicant would find it, but it is a normal requirement for 90+ day stays. I wonder if many folks will fall foul of this.
there is nothing required for a 90 day report at MOST offices but proof you're staying at the address you use on the form when you file. That'd be with the TM30 filed by you or your land lord.
'suggestions' are frankly just the 'noise from the internet' and there is no actual substance to any of it - yet.
It seems unlikely that an immigration officer at an airport will ask for supporting documents.
If one needed to visit immigration for any reason, it's quite possible that they would require supporting documents. The '3 month reporting' required by some visa types is an example of this.
Getting the visa is one thing. Ensuring that it remains valid and able to withstand scrutiny is another. That burden lies with the holder of the visa, not the consulate involved with originally issuing it.
