What details should I provide when applying for a retirement O visa E-Visa for Thailand?

Admin - I had to replace the original Pic in my post as it's revealed some personal information.

I am trying to apply for an E-Visa for a retirement visa type O.

1. What should I put in the "intended date of departure"? After all, I plan to retire and have no date for return.

2. What should I put in the date of arrival? I am not sure about the exact day; I just know an approximation. I am waiting for the visa first.

3. Accommodation: I plan to stay in a hotel for a week or two and hope in that time to find an apartment /condo. I haven't made a reservation yet, as I am waiting for my visa to be confirmed yet.

Should I put the name of that hotel?

4. What about the flight number? I don't plan to book a flight before I get the visa.

What is the cost for a non o visa applied outside Thailand ,,im struggling to find the information on that visa ,.I can find the information on the OA or OX on both the Saigon and Hanoi embassy pages …it says 80 USD

80 USD is the cost of the 90 day Non-O visa in Burma Cambodia, Vietnam, it's 2000baht in Vientiane/Savannakhet Lao,

1 - Date of departure from thailand should be 90 days after your arrival as you can only get stamped in for that long on a Non-O visa

2 - Date of arrival has to be within 3 months from your application date because that is the validity of the visa

3 - Put the name of a hotel where you might stay

4 - Flight numbers don't change (same flight lands the same time every day pretty much) so put a flight in that you might book

MAKE up dates, but whether you're going to leave or not, your Non O visa is for 90 days so have your departure within that

Thanks for the info.

Another question regarding the 'Document indicating current location". I'm not from an English speaking country so the official document is in a local language. They do not offer documents in English. So should I upload what I have or need a translated notarized statement from a lawyer verifying the document?

upload what you have

the document that indicates current location is your visa/entry stamp (and in the case of taiwan that paper you get from the immgiration office that they outline)
