TLDR : Answer Summary
If you are a US passport holder transferring an extension stamp based on retirement, in addition to the form filled out and copies of all pages in your old passport, you should bring both your old and new passports to your local Thai Immigration office. It's also important to ensure that you have a TM30 address registration on file with Thai Immigration.

Arrived back in Thailand last Thursday with both passports as renewed in Australia. Did a transfer next day (Friday) at Korat immigration. Did the TM30 first. No issues. IO told me when I renew my Extension of Stay to bring both old and new passports with me.
