I need it for my driving lesson application here in BKK.

No need for 90 day report at Chaeng Watthana, I applied for a residence certificate without 90 day report last month on 4th day of visit to Thailand.
No photos needed.

Printout of TM30 - property owner needs to have submitted blue book pages about property and owner to TM30 property registration website/paper TM30 submission at Chaeng Watthana.
Copies of passport bio page, visa and entry stamp.
I didn't show a lease agreement or yellow tabien baan.
Certificate arrived in the post 1 week later.

I think Bangkok needs you to have submitted a 90 day report before they will issue you with one.
If you live in Bangkok, you will need to apply for your Residence Certificate at the main Immigration. If you are based outside of Bangkok you should be able to get it from your local immigration office.

Passport
2 x passport-sized photos
Copy of your lease agreement/house registration book (yellow Tabien baan)
Copy of receipt of address notification (90) day report
