Even though you have all the requirements and you don't have visa you can't enter here in thailand. Much better apply for a tourist visa in royal Thai embassy Makati. The immigration in the Philippines will not allow you to enter the country (Thailand) if you don't have visa. They don't give visa on arrival.

How do I correctly fill out the affidavit for a certificate of residency from the US Embassy in Thailand for my driver's license?

Just go to your nearest immigration office and ask for resident certificate just need to pay 100-300 baht but actually it's free. If you don't want to be questioned just pay them.
