Just print it off complete that's what they asked for at all airports UK Dbx and Bkk they also want to see your RT PCR test document

TM8 IS the application form for re entry permit for those on retirement extensions etc. Not needed for tourism. The T8 seems defunct now Thailand Pass is in operation. At Thailand pass check in at Bkk Airport there is a list of things which there ask if you are suffering with. Similar to those on the old T8.
