Form T8 is a free online download to complete yourself for airline check in and arrival and it is not a health declaration form. Although at the end it does ask if u r displaying certain symptoms. Here you go. I will 'process' it, whatever that means, for just $10....lol:
When you come from Africa into Bangkok. There is health control area where you have to present your yellow fever certification and as they call it at the airport a health declaration form (T8).
Been around for ages its for traceability with your seat number.
Its not a new thing thats been introduced for covid hence those questions its more geared to yellow fever, cholera and malaria
