I applied and received my non-immigrant "O" visa based on marriage and the first issuance was for 90 days, which was entirely in correspondence with what I had read.  So I was close to expiration for the initial issuance and went to immigration with all of the required documents and was told that the application would go to Chiang Mai and I was to return in one month to be issued the remaining 11 months of visa.  The one month delay threw me for a loop.  Is this normal or a red flag?

Completely normal for marriage. This will happen every year. During this time they may come do a home visit to confirm you live with your wife where you said you do, interview your neighbors to ask if they see you together, etc.

Just return at the date they provided you with your passport and 99.9% of the time you'll be in and out with your new extension.
