How can I submit my PCR test booking when using a SHA+ hotel in Phuket?

Phuket Thailand pass query..  I have an Sha+ hotel but it doesn’t include the PCR test at Phuket Airport which I have to book separately 

There is only one place to upload the hotel confirmation and no other place to upload the PCR test booking.  How have others solved this please?
