For walk in embassies, anybody has seen a rejection after they accepted your documents, checked them and collected your passport as well as your fee payment?

The disclaimer is on every embassy notice, but in practice, they may not do that. Unless you're aware of someone who applied in Indonesia, paid, had his visa denied and wasn't given a refund. So far, I have not heard of this happening anywhere except the online e-visa platform.
Each consulate is different, but in practice, embassies and consulates that still allow in person applications generally won't take your money unless you've been approved (or conversely will refund it if you're denied).

Now once consulates go to eVisa that's a different ball game, you get rejected you lose the money and pay again to apply there is no "do overs" on the eVisa system
they do a rough check of the documents in Vientiane/Savannakhet but they had a HUGE problem of people turning in fake bank documents so now they vet them a little better (sometimes making you log in to your account and show the balance when you apply) and I think not really all that many people are rejected/refunded up there in the grand scheme of things
