Fee for the single reentry permit is 1,000 Thai baht... But the counter may ask for 1,200 as they have helped you complete the form and copy.

It's in International Departure 2 after security screening. Tell them you need a re-entry permit and if no one is at the desk they'll get someone for you.

Yes if you fail to get this your current extension will expire on departure.

or you can get one at the immigration office when you get your extension.

It does NOT add any time to your existing stamp all it does is let you leave/come back on it and be stamped in for the same expiration date .
