Where can I extend my 60-day visa stamp in Thailand for another 30 days?

Hi I have entered Thailand and got a 60 days stamp on arrival where do we extend this for another 30 days is it still at Lak Si Square ?

this form is when you first enter Thailand and must be completed within 24 hours. Its now required at IT Lansing square....Did you not do?
