At the bottom left hand side of all visa application forms, there appears 'Date of Previous Visit to Thailand' -
Do they want the last time you arrived in Thailand?
Or is it your departure date?

So basically if it is your first time visiting Thailand then you leave it blank, otherwise it would be your last arrival stamp date.

My apologies, i forgot to mention that I am applying outside of Thailand - three different visas for three family member - Tourist/ ED/Non O [dependent visa]. Each application form has this question.
