Hello. Am trying to fill out the form for tourist visa at [members only]/ but the form is asking for many things that I don't think are actually required. For example it's asking for a criminal record check and for a medical certificate. 

I have never been asked those when entering thailand. 

It's also asking for for proof of 14 days SAQ but the new rule is just 1 day test & go. 

Am I on the right site? Do travelers need to acquire their tourist visa before departing or is it possible to have a visa on arrival like before?

Something is not correct, I believe you picked the wrong choice when you started applying.

If you're just wanting a 'regular' single entry tourist visa you DO NOT want that top STV choice you want THIS choice.
