TLDR : Answer Summary
A 5-month-old baby needs to use the Italian passport for exit from Thailand due to the entry stamp being in that passport. Although the visa expired 2 months ago, the baby may receive an overstay stamp but typically will not incur a fine. Upon returning to Thailand, the baby can enter with the Thai passport, but if traveling by air, either passport can be used at arrival.

If the baby leaves Thailand he need to use the Italian passport as the entry stamp is inside this passport. He may get a stamp for overstay but no fine. On return to Thailand use the Thai passport to enter and leave Thailand, if you travel by air you can choose to use the Italian or Thai passport on arrival.
